Chlorine: The Primary Concern
Municipal water treatment uses chlorine or chloramine to kill pathogenic bacteria and make water safe to drink. This is necessary and effective for public health. The same chlorine that prevents waterborne illness, however, reacts with skin tissue during showering in ways that have documented negative effects.
Chlorine is an oxidizing agent. In contact with skin, it oxidizes the lipids in the skin barrier matrix, disrupting the organized lamellar structure that maintains barrier integrity. It also oxidizes proteins in the stratum corneum, altering their structure and reducing barrier function. Regular exposure to chlorinated shower water produces cumulative skin barrier disruption that contributes to dryness, sensitivity, and increased reactivity to other exposures.
Chlorine also affects the skin microbiome — as discussed in our microbiome content, chlorine's broad-spectrum antimicrobial action reaches beneficial commensal bacteria on skin surfaces, contributing to the microbiome disruption that is associated with increased inflammatory skin conditions.
Hard Water Minerals
Hard water — water with elevated calcium and magnesium ion concentrations — is present in a large portion of US households. These dissolved minerals react with soap molecules to form calcium and magnesium stearate — the insoluble compounds responsible for soap scum on shower surfaces and skin.
Research has found associations between hard water exposure and eczema severity. A clinical study in the UK found that children in hard water areas had significantly higher rates of eczema than children in soft water areas. The proposed mechanism involves the calcium deposits left on skin after hard water washing, which have been shown to directly irritate skin and disrupt barrier function at the concentrations found in hard water household use.
Fluoride
Fluoride in municipal water at typical treatment concentrations (0.7 mg/L in the US) has not been shown to cause direct skin harm at shower exposure levels. However, fluoride is absorbed through skin during showering, and people who follow Dr. Sebi's or Barbara O'Neill's protocols around reducing fluoride exposure often prefer filtered water for this reason alongside dietary fluoride reduction.
Heavy Metals
Older plumbing systems — common in cities like Philadelphia — can introduce lead and other heavy metals into household water from pipe corrosion. Lead absorbs through skin in small amounts. For residents of cities with aging infrastructure, shower water filtration that addresses heavy metals is a meaningful health consideration beyond just skin care.
