Blisters are among the most common acute skin injuries in athletics, capable of ending an event or sidelining training despite being relatively minor injuries. Understanding how they actually form clarifies why certain prevention methods work and why some common treatment approaches make things worse.
How Blisters Actually Form
Blisters form from a combination of friction, heat, and moisture. Repeated rubbing creates shear force between skin layers, and when this force exceeds what the skin can tolerate, the layers separate and fluid fills the space between them. This is fundamentally the same friction mechanism behind chafing discussed in our dedicated article, but occurring in the thicker skin of the feet and hands where the layers separate rather than wear away at the surface.
Why Moisture Matters So Much
Damp skin has a dramatically higher friction coefficient than dry skin, which is why moisture is such a powerful blister accelerant. Feet that sweat heavily inside shoes, or that get wet from rain or stream crossings, blister far more readily than dry feet under the same mechanical stress. Prevention: Managing the Three Factors
Reducing friction through properly fitted footwear and moisture-wicking socks addresses the mechanical cause. Managing moisture through appropriate sock material, foot powder, or antiperspirant on blister-prone areas reduces the moisture that accelerates blistering. And reducing heat through adequate footwear ventilation addresses the third factor, though this is often the hardest to control during intense effort.
Some athletes use lubricants or friction-reducing products on known blister-prone areas, similar to the anti-chafe approach discussed in our chafing content. Others prefer to address friction through better-fitted equipment rather than products, since a lubricant can wear off during long efforts.
The Hot Spot Warning
Blisters usually announce themselves before they form, as a hot spot, a localized area of warmth and mild discomfort that signals excessive friction. Addressing a hot spot immediately, by adjusting footwear, adding a barrier, or changing socks, often prevents the blister from forming at all. Ignoring it almost guarantees a blister. This early intervention is far more effective than any treatment after the blister has developed.
To Pop or Not to Pop
Intact blisters provide a natural sterile barrier over the healing skin beneath, and leaving them intact when possible reduces infection risk. When a blister is large, painful, or in a location where it will certainly burst on its own, careful drainage with a sterilized needle while leaving the overlying skin intact as a protective cover is the generally recommended approach. The overlying skin should not be removed, as it protects the vulnerable new skin underneath.
