Blister Prevention for Ultramarathon Runners: A Complete Guide

Blisters end more ultramarathons than injury, weather, and training failures combined. They're also almost entirely preventable — if you treat blister prevention as the serious preparation it deserves.

Why Blisters Form

Blisters are the skin's response to shear force — the lateral movement between skin layers caused by friction. In ultramarathons the problem isn't a single friction event — it's cumulative, repeated shear force over hours, combined with moisture that softens skin and reduces its resistance to shear.

The Four Factors You Can Control

Moisture management. Wet feet blister faster than dry feet. Technical running socks made from merino wool or synthetic moisture-wicking materials reduce internal moisture. At aid stations, change into dry socks if you've done any wet crossings.

Fit and friction. A shoe that fits well on a 10-mile run may develop hot spots at mile 40 as feet swell. Consider shoes with slightly more room in the toebox than you'd wear for shorter runs.

Skin preparation. Skin that has been conditioned — well-moisturized, not dried out from harsh soap — is more resistant to shear force. Two weeks of natural soap like our Pine Tar Rugged Bar and daily moisturizing makes a measurable difference in how long your foot skin holds up.

Protective application. Balm applied to known hot spots before racing reduces shear force by decreasing friction between skin and sock.

When a Blister Forms Mid-Race

A small, intact blister is best left alone. A large blister causing significant gait change should be drained with a sterile needle at an aid station, fluid pressed out gently, and the overlying skin left in place as a natural bandage.

A ruptured blister is an open wound. Clean it with Tea Tree Antibacterial Bar Soap, cover and protect, and monitor for infection.

Post-Race Foot Recovery

Feet take the most punishment in any ultra. After finishing, elevate them. Once ready to shower, use a gentle natural soap that won't further irritate raw skin. Apply balm to any blistered or raw areas. The runners who come back to the start line healthy are the ones who take foot care as seriously as training.
